Objective: Smartphone-based gait evaluation of 3 different movement disorders.
Background: The diagnostic tools to differentiate between different movement disorders are still limited.
Method: Gait evaluation by using a smartphone application was performed in patients diagnosed with Parkinson’s disease (PD), normal pressure hydrocephalus (NPH) and psychiatric patients on neuroleptics (PPN), compared to healthy controls (HC).
Results: A total number of 458 participants (268 males) were included: 101 PD patients (age 68.3±11.0 years), 37 PPN (age 46.2±12.7 years), 127 NPH patients (age 77.5±6.0 years) and 193 controls (54.7±18.2 years). NPH (7.23 sec) and PD (6.94 sec) patients walked much slower than PPN (3.21 sec) and HC (3.09 sec) in the straight-walking TUG segment. ML-sway was lowest in the PPN group (5.30cm), compared to PD (6.95cm, p=0.02) and NPH (7.05cm, p=0.01). AP-sway was smallest in NPH and PPN (5.06cm and 5.11, respectively, p>0.05), followed by PD (6.28cm, p=0.02 compared to NPH) and greatest in the HC (8.02cm, p<0.001 for all comparisons). NPH had shortest step length and lowest step correlation, followed by PD, PPN and HC. PPN had the highest AP step correlation (0.44), compared to PD and NPH (0.24 and 0.23, respectively, p<0.001). Standing up and sitting down times and step length were the most discriminating variables among the 3 disorders.
Conclusion: PPN had distinctly small ML-sway while their gait speed is faster and their steps are highly correlated than PD and NPH.
