Improved sleep quality evaluated by Parkinson’s disease sleep scale after deep brain stimulation of globus pallidus internus

Objective: To elucidate the sleep quality after deep brain stimulation of globus pallidus internus (GPi-DBS) for Parkinson’s disease.

Background: Sleep Disturbance in Parkinson’s disease and its improvement after deep brain stimulation of subthalamic nucleus (STN-DBS) are frequently reported. The effect of GPi-DBS on the sleep has not been as clear as that of STN-DBS.

Methods: We evaluated the baseline sleep quality of 18 patients (age: 62.2 ± 8.3 years; males: 6, females: 12) using Parkinson’s disease sleep scale (PDSS). We obtained postoperative PDSS profile from 14 of them (age: 61.2 ± 7.7 years; males: 5, females: 9; 16.2 ± 21.3 months after surgery). Clinical profiles were evaluated by Unified Parkinson’s disease rating scale (UPDRS) motor score, mini-mental state examination (MMSE) and levodopa equivalent daily dose (LEDD). Statistical significance was evaluated by Wilcoxon matched-pairs signed rank test and Pearson’s correlation test.

Results: The total PDSS score increased from 100.9 ± 14.0 to 111.8 ± 17.1 after GPi-DBS. Among each item, only the item 8 (question about night urination) improved significantly.

The baseline PDSS total score correlated with disease duration (Pearson r = -0.51, p = 0.03).

Conclusions:  Sleep quality evaluated by PDSS improved after GPi-DBS. DBS could alleviate nocturnal autonomic symptoms.
