Clock Drawing Test does not Differentiate Alzheimer’s Disease from Parkinson’s Disease Dementia

Objective: Our focus was on comparing the performance of CDT and MMSE in Parkinson’s Disease Dementia (PDD) and Alzheimer’s Disease (AD) patients.

Background: The mini mental state examination (MMSE) and clock drawing test (CDT) have been widely used in clinical examination for detecting indi­viduals suspected of having cognitive impair­ment. Our focus was on comparing the performance of CDT and MMSE in Parkinson’s Disease Dementia (PDD) and Alzheimer’s Disease (AD) patients.

Method: We assessed performance on MMSE and CDT in 24 patients with PDD and 30 patients with AD. Age, gender, disease duration, years of education, CDT and MMSE scores of the participants were recorded.

Results: According to the results of the comparison of the 24 PDD patients (14 males, 10 females) and 30 AD patients (16 males, 14 females) there was no significant difference between two groups in terms of mean age (AD=75.73±5,57, PD=73,54±7,77), mean years of education (AD=11.7±4.56, PD=10.29±4.74) and gender distribution. Also, two groups did not differ on the MMSE and CDT scores (respectively p= 0,627; p= 0,618).

Conclusion: Considering different cognitive profiles of AD and PDD patients, it is known that CDT is more sensitive in PDD and deteriorates in early stages. However, no significant statistical difference in CDT scores was detected between our groups despite similar disease duration and MMSE scores.
