Clinical and neurological features of the course of parkinsonism in the presence of chronic renal disease

Objective: To study the clinical and neurological features of the course of parkinsonism in chronic renal disease.

Background: Under our supervision were 30 patients with chronic kidney disease, who have parkinsonism, the control group consisted of healthy people of a similar age. The average age (10 women and 20 men) aged 40-57 years (average age 48.5 ± 1.2 years).

Method: To study of neurological status, level of lactate dehydrogenase (LDH-4), hemoglobin (Hb), serum albumin (Alb), magnetic resonance imaging (MRI) of the brain.

Results: Patients in the main group have  headaches- (100%), periodic dizziness- (73%), postural instability- (68.3%), seizures-57%, cognitive and visual impairment -83%, also a violation sense of smell-43%, sleep disturbance-37.7%, excessive sweating-23.9%. In the neurostatus, in 95% prevailed the stiffness and puppet gait, amimia-45% lack of physiological synkinesia, 37.1% rest tremor, and disorders of higher cortical structures-37.8%. Analysis of MRI data showed the presence of changes in 100% of cases, mainly in the posterior regions of the brain and in the basal ganglia +. The depth of neurological impairment correlated with an increase in LDH-4 -362 ± 3.7 U / L, a decrease in hemoglobin (Hb) -72.3 ± 0.4 g / L and serum albumin (Alb) -26.3 ± 0.4 g / l.

Conclusion: Parkinsonism can develop in patients with chronic renal disease, the severity and course of the disease while correlating with an increase in the level of LDH-4, a decrease in the level of hemoglobin (Hb) and serum albumin.
