Prevalence and Characteristics of Postural Deformities in Thai Patients with Parkinson’s Disease

Objective: We aimed to investigate the prevalence, clinical characteristics and quality of life (QoL) in Thai Parkinson’s disease (PD) patients with postural deformities (PostD) and without postural deformities (non-PostD).

Background: PostD are important and likely lead to long term disabilities in PD patients. Understanding of these conditions would be beneficial for the effective management in the future.

Method: PostD in 100 Thai PD patients were evaluated by using a computerized measuring program which measured the angles of neck flexion (NF), upper back flexion (UBF), total back flexion (TBF), and truncal lateral flexion (LF), from patients’ photographs. Antecollis, total camptocormia, upper camptocormia, and Pisa syndrome were defined by the angles equal or greater than 45, 30, 45, and 10 degrees from the reference lines, respectively. Clinical characteristics, UPDRS, gait velocity, cadence, and QoL (PDQ8) were assessed in all patients.

Results: Of all patients, 13% had PostD (10% camptocormia, 1% antecollis, and 1% Pisa Syndrome). The mean age of patients with PostD was 66.4±12.3 years, with the median disease duration of 6 years. PostD developed approximately 2 years after the onset of PD. After multivariate analysis, patients with PostD had a significantly higher modified Hoehn and Yahr scale (p=0.008), higher proportion of head compensation (p=0.008), and lower gait velocity (p=0.017), compared to non-PostD patients. However, the QoL did not exhibit a significant difference between 2 groups.

Conclusion: The most common PostD in Thai PD patients was camptocormia. PostD were significantly associated with higher severity of disease and slower gait velocity. A larger study involving more PostD patients may explicit a relation to QoL in PD patients.
