Autonomic Symptoms as Predictors of Quality of Life in Parkinson’s Disease: A 5-year follow up study

Objective: The aim of this study was to investigate whether the autonomic symptoms could predict the quality of life of patients with PD.

Background: Parkinson’s disease (PD) is a multi-system neurodegenerative disorder characterized by motor and non-motor symptoms. Autonomic nervous system (AMS) disorders are part of the spectrum of non-motor symptoms in PD, which are more difficult to manage than typical motor symptoms of PD.

Method: This is a 5-year retrospective observational cohort study of 205 patients with PD who were recruited from a private geriatric center between January 2015 and December 2020. The composite autonomic symptom scale 31 (COMPASS 31) was used to assess the autonomic symptoms, Hoehn & Yahr (H / Y) to measure the level of disability and the greek version of PDQ-39 to measure patients’ quality of life. We used the Spearman’s rank correlation and multiple linear regression analyses to evaluate the relationship between COMPASS 31regions and quality of life dimensions.

Results: A total of 205 patients were examined, aged 72 ± 9.6 years. 74 were women (36.1%) and 131 men (63.95). The M.O. years of disease duration  is 10 years and the M.O. of Hoehn & Yahr (H / Y) is 2.5. The final stepwise multiple linear regression model showed that male sex (standard partial regression coefficient β = 0.121, P < 0.05) and baseline gastrointestinal (β = 0.362, P < 0.01) and vasomotor (β = 0.561, P < 0.01) scores significantly predicted the PDQ-39-SI scores at the 5-year follow-up.

Conclusion: At this first 5-year follow-up study, gastrointestinal and vasomotor symptoms may be useful for predicting the quality of life of patients with PD.
