Comparative study patients with different clinical forms of Parkinson’s disease

Objective: To study the cognitive, emotional status, sleep disturbances in patients with akinetic-rigid and tremor form of Parkinson’s disease (PD).

Background: Parkinson’s disease (PD) – one of the most common neurological disease of elderly. Among the non-motor symptoms distinguish the cognitive, sensory and emotional disorders. Non-motor symptoms significantly affect the quality of life of patients.

Methods: To assess neurological and cognitive status used the following scale: Unified PD Rating Scale (UPDRS), Mini-Mental State Examination (MMSE), Frontal Assessment Batter (FAB). The degree of anxiety and depression was studied by Beck Depression Inventory (BDI) and Spielberger test (ST). Feature Sleep studied with the help of the scale of daytime sleepiness by Epworth.

Results: The comparative aspect studied cognitive, emotional state and sleep disorders in 21 patients with akinetic-rigid form of Parkinson’s disease and in 12 patients with tremor form of the disease. The mean age was 60,5 ± 6,9 years. The total combined score on the UPDRS in patients with akinetic-rigid form of PD was 62,3 ± 19,4, with tremor – 58,4 ± 10,72. There was a significant (p <0.05), the worst performance MMSE and FAB in patients with akinetic-rigid form of PD (25,8 ± 2,4 and 14,7 ± 3,7) relative to the group of patients with tremor form of (28,7 ± 1,1 and 16,8 ± 1,8). Comparative analysis found that patients with tremor form of the disease has a high degree of anxiety. The average value of reactive anxiety was 48,2 ± 11,2 points, personal anxiety – 49,7 ± 8,1 points, which is significantly higher than the values of these scales in patients of the comparison group. Average on a scale of Beck in patients with akinetic-rigid form of PD 20,7 ± 9,6 points, which is significantly greater than the tremor form of the disease. According to the Epworth Sleepiness Scale in patients with akinetic-rigid form of the disease was marked by moderate daytime sleepiness (mean score Sleepiness Scale Epworth 9,1 ± 4,9), patients with a trembling PD form showed no signs of excessive daytime sleepiness.

Conclusions: Cognitive impairment and depression are significantly more expressed in patients of the akinetic-rigid form of Parkinson’s disease, but anxiety more expressed in patients with tremor form of the disease.
