Objective: To examine the use of oculometric measures (OMs) as a tool to differentiate patients with Leucine-Rich Repeat Kinase (LRRK2) – associated (L2PD) and idiopathic Parkinson’s disease (iPD).
Background: Abnormalities of OMs were widely described in PD patients during the last decades. However, their potential in differentiating patients with LRRK2 genetic mutation from idiopathic PD patients is still lacking, potentially used as a pre-screening tool in clinical trials.
Method: L2PD patients (18 patients, mean age 66.3±11.2 years, 10 females, MDS-UPDRS score range 29.5±13.0), and 108 matched patients with iPD were enrolled. All patients were evaluated using an oculometric software-based platform using computer vision and deep learning algorithms. Comparisons of OMs between groups were performed using T-Test.
Results: Oculometric measures of saccadic eye movements were different between L2PD patients compared with iPD patients (Pro-saccades reaction time (ms): 208.1±31.1 vs. 254.2±62.4, p<0.01; variability of pro-saccadic reaction time: 65.9±25.9 vs. 88.4±37.8 p<0.05; and variability of anti-saccadic reaction time: 76.6±36.1 vs. 108.4±54.6, p<0.05). In addition, measures of pro-saccadic reaction accuracy were also found different between the 2 groups (4.1±1.4 vs. 5.4±2.4, p<0.05).
Conclusion: Oculometric patterns of PD patients were found to be statistically significantly different between L2PD and iPD patients, for both saccadic reaction time and accuracy. Future clinical trials can potentially use oculometric measures as an additional stratification and pre-screening tool in trials involving patients with genetic mutations, as well as combining different OMs to create a combined score which can assist in a more detailed analysis for each individual patient.
