Objective: To systematically evaluate the effectiveness of HBEI on motor symptoms, balance, and overall well-being in individuals with mild to moderate PD, providing insights into optimal exercise modalities for clinical practice.
Background: Parkinson’s disease (PD) is a progressive neurodegenerative disorder significantly impacting motor function and quality of life. Home-based exercise interventions (HBEI) have emerged as promising adjunctive therapies, yet their comparative efficacy remains unclear.
Method: A comprehensive literature search was conducted across Cochrane, PubMed, Scopus, and Web of Science databases. Inclusion criteria encompassed randomized controlled trials (RCTs) and longitudinal studies assessing HBEI in PD patients. Outcomes analyzed included gait speed, balance, Unified Parkinson’s Disease Rating Scale (UPDRS) scores, and quality of life (QoL) measures. Risk of bias was assessed using the Cochrane Risk of Bias tool.
Results: The analysis included 154 studies with a total of 7,837 participants. Home-based interventions incorporating resistance training, balance exercises, and gait training demonstrated significant improvements in motor function (UPDRS-III mean difference -5.76, 95% CI: -9.78 to -1.74) and QoL (PDQ-39 mean difference -4.72). Dance-based interventions and tele-exercise programs yielded the most consistent benefits in gait and balance improvements. Notably, adherence rates exceeded 85% across studies, with minimal adverse events reported.
Conclusion: HBEI significantly enhance motor function, balance, and QoL in mild to moderate PD. Their accessibility, cost-effectiveness, and feasibility support broader integration into clinical practice. Future research should focus on optimizing exercise modalities and long-term adherence strategies to maximize therapeutic benefits.
