Evaluation of Care delivery model for integrated care and self-management for people with Parkinson’s disease with Deep Brain Stimulation

Objective: To evaluate the implementation and impact potential of a pragmatic network for PD care, the Integrated Parkinson Care Network (IPCN) for patients with chronic DBS.

Background: Parkinson’s disease (PD) has a myriad of motor and non-motor symptoms. Deep Brain Stimulation (DBS) can have dramatic impact in the lives of people with PD (PwP) with motor complications or tremor refractory to medical treatment. Nevertheless, care delivery for this group of patients remains complex. There is a lack of evidence on the impact of integrated care delivery models in PwP and chronic DBS.

Method: This is a subgroup analyses of a 6-month, pre–post design, single-centre, phase 2 study to assess a patient-centred care model based on integrated care, self-management support, and technology-enabled care (IPCN), for the group of study participants with active chronic DBS.

Results: We identified 22 study participants with active chronic DBS (all with bilateral Subthalamic Nucleus) . The mean age was 63.9 ± 7.6 years, 16 were males. The mean disease duration was 15.3± 7.0 years. Overall, the top three care priorities were speech (54.5%), mobility (40.9%) and mood troubles (31.8%). The most commonly used resources were speech-language pathology (78.3%) and physiotherapy (43.4%). There was a positive change in quality of life (PDQ-8: 7.1, 95%CI:1.76 – 12.45), perception of support for chronic care (PACIC: 0.85; 95% CI: 0.4 to 1.2) and self-management (5As: 0.77; 95%CI: 0.39 to 1.15). Improvement in the clinician global impression of change was reported in 86.4% of the cohort.

Conclusion: The IPCN is a care delivery model that may further enhance care for people with PD and DBS. The current study showed its feasibility and warrants further evaluation.
