Evaluation of the short-term and long-term effects of an intensive multidisciplinary collective rehabilitation program over 5 weeks in Parkinson’s patients followed in Lyon

Objective: We studied the value of collective (8 patients per group) intensive multidisciplinary collective rehabilitation in the short and long term.

Background: Numerous studies have proved the beneficial effects of physical exercise in Parkinson’s disease, with an improvement in quality of life, symptoms and a decrease in functional disability.

Methods: The rehabilitation program was based on the principles of the “big and loud” LSVT: five weeks of physical therapy and exercise, with 4-5 daily sessions, five days a week. Patients were evaluated at baseline, at the end of the program (T1), at one year (T2). The parameters studied were: GABS, test walk (T6m), sit-stand test (TAD), a daily life activity scale (ADL), a functional writing scale , the vocal intensity during reading (I reading), the maximal vocal intensity (I phonation), the duration of  loud “a” (Tmax).

Results: 39 patients were enrolled between November 2014 and August 2015. The evaluation at T1 involved 36 patients. All parameters evaluated were significantly improved (p <0.05) except T6m (p = 0.0547). At T2, 29 patients could be evaluated. All parameters were improved but only the GABS, the I phonation and the I reading were significantly modified (p <0.05). In addition, the number of neurological consultations was significantly reduced (p <0.05) over the 12 months following the end of the rehabilitation/ 

Conclusions: This pilot study confirms the high potential for recovery in fluctuating PD patients with no major cognitive impairment, with a persistent effect at 12 months. Our results also confirm the feasibility and effectiveness of the LSVT method in groups of 8 patients. Our results must nevertheless be reproduced in a larger study with a control group to validate the interest of this approach.
