Evaluation of the surface based measures to discriminate progressive supranuclear palsy from corticobasal syndrome

Objective: To evaluate structural gray matter (GM) changes tested by surface-based measures of cortical thickness (CTh), surface area (SA) and volume (VOL), in order to characterize the morphometric measures in Corticobasal syndrome (CBS) and Progressive Supranuclear Palsy (PSP) patients.

Background: CBS and PSP are atypical parkinsonism, pathologically characterized by tau protein accumulation in several brain regions. Since, CBS and PSP patients show similar clinical features it would be useful to differentiate the two disorders.

Methods: We recruited 11 CBS patients (3 males and 8 females, mean age±SD: 68±6.9) and 19 PSP patients (11 males and 8 females mean age±SD: 70.93±5.2) who were diagnosed on the basis of standard clinical criteria at the Movement Disorders Clinic, Department of Neurology and Psychiatry, Sapienza University of Rome, Italy. Fourteen age matched healthy subjects (HS) (7 males and 7 females, mean age±SD: 69.3±5.2) were also recruited.The patients’ clinical features were scored by standardized clinical scales. T1-weighted and diffusion tensor images (DTI) were acquired using a 3T magnet and analysed using FreeSurfer. We compared CTh, surface area and cortical volumes among the patient and HS groups.

Results: Significant volume loss were observed in both CBS and PSP patients group as compared to HS. No significant volume differences were found between the two patient groups. Comparing CTh and surface areas in CBS and PSP patients with HS, we found significant decrease in CTh in CBS group and significant decrease of surface area in PSP patients. In comparison to PSP, CBS patients had a greater decrease of CTh in bilateral precentral gyrus, left temporal pole left precuneus and in the right superior parietal brain regions .

Conclusions: CTh might be used as a useful measurement in discriminating patients with PSP and CBS.
