Objective: To describe the incorporation of a prescribing pharmacist into a Multi-disciplinary Parkinson’s clinic based in a University Hospital
Background: Evidence shows that a multidisciplinary approach to disease management offers the most holistic, patient-centred care for People with Parkinson’s(PwP). Whilst medical, nursing, physiotherapy and occupational therapy are fairly standard MDT members, there are relatively few services locally who have embedded pharmacy specialism in their PD service.
Methods: For a year, a senior pharmacist joined the PD team in a training role during which she developed clinical skills and knowledge relating to diagnosis and management of PD, and at the end of which she achieved Independent Prescriber status. Following this, the SIPP integrated into the clinic and developed a structured consultation pro forma, using this to independently consult PwP with a focus on their Parkinson’s-related medications. Through team reflection and patient feedback we obtained qualitative data on the perception of this new role
Results: Patient feedback was overwhelmingly positive and all felt that their general care had been enhanced by having a least one consultation with a strong focus on medication review. Team reflection was similar, with all agreeing that this role brought a helpful new dimension to the holistic management for People with Parkinson’s.
Conclusions: A number of additional benefits ensued such as an enhanced relationship with the pharmacy department which facilitated i) the development of a Health Board Guideline for management of Parkinson’s in inpatients and ii)improved access to medicines information queries. The structured consultation provides consistency and uniformity to medicines reviews in Parkinson’s. We conclude that the addition of a pharmacist to the Parkinson’s MDT enhances the care offered to PwP
