IParkours : Role of the advanced practice nurse (APN) in the support/follow-up of parkinsonian patients.

Objective: Specify the contribution of APN in the follow up of patient (prescription, follow-up rate, health literacy).

Expected evolution to increase the impact of the physician.

Fluidify the circulation of multi-professional information 

Improving the care of Parkinson’s patients: accessibility of clinical expertise.

Background: The APN,as a new intermediate healthcare professional, has been seeking its place and role in the French healthcare system since 4 years now.

The notion of chronic stable pathologies includes neurological illnesses and more precisely Parkinson disease.

Statistics show a sharp increase in the number of patients with this disease, health professionals must be then capable to propose solutions in communication with patient’s associations to help set a clear course for Parkinson’s patients.

Method: The method used is an intra-professional focus group of 12 APNs from different types of structures and other nurses participating in the patient journey: 1 liberal nurse, 1 home care nurse and 1 parkinson expert center nurse.

Thematic discussion on 4 different subjects: Transmissions, Direct access, Monitoring, Transversal missions.

One hour of debate per subject.

Audio and video recording.

Results: There is a real lack of communication between the different professionals working with the patient.

The journey of the Parkinson’s patient is currently not structured : patients have difficulty accessing to health care.

It seems that the APN has a key role to play, with the possibility of modifying and adapting oral and device treatments , or after additional training, validating modifications and verification of medical devices (deep brain stimulation).

The APN could make this central link with the various professionals, while being a support for the rehabilitation strategy and helping the increase in knowledge and decision-making for patients.

This agreement seems even more essential in the city, because of the shortage of professionals.

The successful experience of a liberal APN highlights the need to develop this mode of exercise.

Conclusion: The APN has the potential to respond to the problems of Parkinsonian patients.

Its field of prescription is very limited and limiting to leave a full measure with the patient.

Its impact in the management of therapies, its overall clinical knowledge and its availability would help the fluidity of information, compliance and understanding of therapeutic strategies.
