Objective: To relate the non-motor offs theory , sensory signs and levodopa use with neurophysiiology  terms in PD

Background: Peripheral Nervous System disease concept in PD has been recognized as a growing interest issue nowadays , origined several years ago in Cory Toth , D Zochodne and O Suchowersky series ( Ann Neurol 2010 , MD 2008 ) , driving along a controversy in those publications in those years ( 2010 ) .Results indicated up to 55% of prevaklence of PNin iPD , with age-ranges , UPDrs , Hcy lebvels and cumulated L-dopa in upper limits.

Methods: Comparative analysis among :

– American-Brazilean group series ( Gondim F ) ( MD Congress 2009 )

– Weinberger .Inzellberg references over sensorial -temporal discrimination in PD ( 1992- 2010 )

—Controls with peripheral disease in SCA and AOA 2 ( ataxia with oculomotor apraxia ) isuggested by Madrid group in MD congress 2009 ( Bermejo F )

Results: It is posible to correlate ataxia figues : neuronopathy 49 % ( Bart 2004 ) , dying-back 30 % , with small-fiber disease absence in PD ( Calabressi – Chicago 2008 )

Conclusions: Peripheral disease in PD is not related to age , UPDRS score , H &Y , evolutive type nor non-motor offs progression. Levodopa cumulative effect acts as co-modulator factor , disclosed by Yebenes et al two decades ago ( as neuroplasticity factor role ) and confirmed by WallerianDegeneration models : axonal prototype and soma prototype ( 2010 )perfectly applyable to PNS / PD topics
