Prevalence of REM sleep behavior disorder and sleep-disordered breathing in multiple system atrophy: A meta-analysis

Objective: This study aimed to evaluate the prevalence of rapid eye movement sleep behavior disorder (RBD), stridor, sleep-disordered breathing and periodic limb movements in sleep (PLMS) in patients with multiple system atrophy (MSA).

Background: Sleep disorders are common manifestations in MSA and include fragmented sleep, excessive daytime sleepiness and RBD. RBD and nocturnal stridor are considered early markers and may be the initial manifestation of the disease before the onset of motor symptoms. We performed a systematic review and meta-analysis of published studies on the prevalence of RBD and sleep-disordered breathing in MSA.

Methods: The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines for meta-analysis was followed. Studies on RBD symptoms, sleep-disordered breathing and MSA were searched using PubMed and Embase databases. All studies were published before February 2018. The primary outcome measure was frequency of RBD and sleep-disordered breathing in MSA as reported in prevalence (%). The pooled prevalence of symptoms and 95 % confidence intervals were obtained by using a random-effects model. Analyses were performed with Comprehensive Meta-Analysis (CMA) software.

Results: The primary search strategy yielded 505 articles of which 37 met the inclusion criteria. The summary prevalence of clinically suspected RBD was 80.9 % (95 % CI, 70.3-88.3 %) in a combined sample of 574 MSA patients. The prevalence of stridor was 32 % (95 % CI, 25.7–39 %); the prevalence of obstructive sleep apnea was 36 % (95 % CI, 28.6–44.1 %); the prevalence of snoring was 63.7 % (95 % CI, 52.7–73.4 %); the PLMS was 39.7 % (95 % CI, 30.9–49.1 %).

Conclusions: RBD, stridor, sleep-disordered breathing and PLMS are common symptoms in patients with MSA. Further studies are needed to understand the natural history and etiology of these symptoms in MSA.
