Objective: Evaluate the effectiveness of delivering specialist Parkinson’s care in community care home settings.

Background: Our drive to improve older people’s care has led to the development of bespoke care home based clinics for those with complex/palliative Parkinson’s in care settings. A pilot study demonstrated small scale efficacy and positive qualitative feedback. However, data remains limited relating to the impact of structured nursing home services for people with Parkinson’s (PwP).

Methods: Following the pilot study of 25 patients (mean age 81) within 4 nursing homes in South Wales, a qualitative analysis led to the establishment of a structured nursing home service. Nursing home clinics commenced in 2013.A follow up analysis was undertaken to establish if the service had improved patient experience and efficiency. Current patient notes were assessed looking at a period between February 2013-July 2014. Frailty scores, H+Y and consultation time were recorded. A Qualitative evaluation form was sent out to the 25 nursing homes where clinics are conducted with results recorded and a cost analysis produced.

Results: A total of 63 PwP (mean age 80) are active in the service (39 female, 24 male). Clinical frailty scores were unchanged from the pilot study 6-8 (Mean 7.04). All 25 evaluation forms were returned. 3 rounds of nursing home visits were performed with a total of 187 reviews. H+Y scores ranged from 4-5. Average length of each consultation 46 minutes. Clinic cost reductions calculated for the above period equate to £63,206 (inclusive of hospital transport). Results from the Qualitative evaluation have revealed 100% satisfaction with the service and a positive impact with provision of services for frail and vulnerable PwP in nursing homes.

Conclusions: Nurse-led care home clinic appear to deliver a subjectively better experience for frail and vulnerable PwP in nursing homes without the time pressures and travel burden associated with hospital based clinics. This service brings specialist care into a setting that is generally underserviced by traditional models.
