Objective: To evaluate MIBG and DAT SPECT for the diagnosis of Parkinson’s disease (PD) and related disorders, we performed 123I-meta-iodobenzylguanzylguanidine (MIBG) myocardial scintigraphy and (123I)-FP-CIT SPECT (DATSPECT) in patients with parkinsonism. In this study, we investigate a correlation between MIBG myocardial scintigraphy and DATSPECT in PD patients.

Background: The diagnosis of PD is based on the neurological findings such as tremor, rigidity, bradykinesia and postural instability. However, other related disorders also showed parkinsonism and it is not easy to diagnose PD, especially, at the time of onset. We applied both of MIBG myocardial scintigraphy and DATSPECT for the diagnosis of PD and related disorders.

Methods: 123 cases were investigated by MIBG myocardial scintigraphy and DATSPECT within one year, between January 2014 and December 2016. In MIBG myocardial scintigraphy, heart to mediastinum ratio (H/M ratio) was applied to show the qualitative value the sympathetic nerve to the heart. In DATSPECT, specific binding ratio (SBR) has been applied in order to evaluate dopamine neurons in the striatum. We analyzed the correlations of the values of both examinations by using the method of quadrisection.

Results: 123 cases were included of PD, and other related disorders such as DLB, PSP, MSA, CBD, and others. 49 patients  were diagnosed with PD by neurologists before both examinations.  These showed the mean H/M ratio was 1.97 (0.81-3.83) and the mean SBR was 3.04 (0.67-5.29). 28 of 49 showed lower values in both examinations (SBR≦4.5, H/M ratio≦2.2). 5 of 49 showed low H/M ratio and normal SBR. On the other hand, it was only one case that showed normal values in both. These findings suggests that when cases with parkinsonism show normal H/M ratio and SBR, the cases will not be PD. Even though DATSPECT shows normal findings, decreasing H/M ratio of MIBG myocardial scintigraphy are useful to diagnose of PD.

Conclusions: The combination study of MIBG myocardial scintigraphy and DATSPECT can be considered useful for the diagnosis of PD and other related disorders.
