The Impact of Non-Motor Symptoms on Quality of Life in Early Parkinson’s Disease: A Prospective Study

Objective: To assess the prevalence and severity of non-motor symptoms (NMS) in early Parkinson’s disease (PD) and their impact on health-related quality of life (HRQoL).

Background: Parkinson’s disease is traditionally defined by motor dysfunction, but non-motor symptoms (NMS) such as depression, cognitive impairment, and sleep disturbances often emerge early, significantly affecting HRQoL【1】. Studies suggest that NMS, rather than motor severity, are the primary contributors to disability【2】. However, the specific NMS that most significantly impact QoL in early PD remain underexplored【3】. Understanding this relationship may guide early interventions【4】.

Method: A prospective observational study was conducted on 150 early-stage PD patients (Hoehn & Yahr ≤ 2). The following standardized tools were used:

• Non-Motor Symptoms Scale (NMSS): To quantify NMS burden【5】.

• Parkinson’s Disease Questionnaire-39 (PDQ-39): To assess HRQoL【6】.

• Montreal Cognitive Assessment (MoCA): To evaluate cognitive function【7】.

• Hamilton Depression Rating Scale (HDRS): To assess depressive symptoms【8】.

Spearman’s correlation evaluated associations between NMS and HRQoL, while multivariate regression analysis identified the most significant predictors of poor QoL.

Results: • Depression and sleep disturbances were the strongest determinants of poor QoL.

• HDRS scores negatively correlated with PDQ-39 total scores (r = 0.72, p < 0.001).

• Sleep disturbances also showed a significant impact (r = 0.65, p < 0.001).

• Cognitive impairment (MoCA scores) was mildly correlated with QoL decline (r = 0.42, p = 0.002).

• Multivariate regression confirmed that depressive symptoms (β = -0.54, p < 0.001) were the most significant predictor of lower HRQoL, followed by sleep disturbances (β = -0.47, p < 0.001).

Conclusion: Non-motor symptoms, particularly depression and sleep disturbances, significantly impair HRQoL in early PD, underscoring the need for early screening and targeted interventions. Routine NMS evaluation should be integrated into PD management to improve patient well-being【9】.
