The impact of STN DBS on kinetic tremor in Parkinson’s disease patients

Objective: The aim of the study is to evaluate the impact of STN DBS on kinetic tremor in PD patients in comparison to pharmacologically treated patients.

Background: Circle movements’ frequency assessment using digitizing tablet can be an useful tool in analysis of kinetic tremor. Subthalamic nucleus deep brain stimulation (STN DBS) can influence on the intensity of both the rest and kinetic tremor.

Methods: The study group consisted of 12 PD patients (7F,5M, mean age 56.7years), after bilateral STN DBS (DBS group). The control group consisted of 10 patients (9F, 1M, mean age 52.8years) treated with pharmacotherapy only (BMT group). Kinetic tremor analysis was performed using WACOM tablet during 3visits (V1,V2,V3) in the mean time of 9±3months in 2 sessions (BMT group/DBS preop visit: BMTon/BMToff) or 4 sessions (DBS postop visits: BMTonDBSon, BMTonDBSoff, BMToffDBSon, BMToffDBSoff). Kinetic tremor evaluation was correlated with UPDRS III score.

Results: There was a significant reduction in kinetic tremor in DBSon vs DBSoff sessions in frequency of 4-6Hz (p<0,05). There was a tendency to 4-6Hz kinetic tremor frequency increase in V3/V2 DBSoffBMToff evaluation, but a significant reduction in V2/V1 DBSoffBMToff assessment in DBS group. There was a progressive but not significant kinetic tremor gain in V3/V2 and V2/V1 evaluation in BMT group.

Conclusions: Kinetic tremor analysis using circle movements’ WACOM tablet can be an useful marker in evaluation of impact of DBS in PD patients.
