Determinant factors of treatment adherence in Parkinson’s disease

Objective: To determine factors predicting treatment adherence in patients with idiopathic Parkinson’s disease.

Background: Parkinson’s disease is the second most common cause of neurodegenerative disease all around the world; and one of the challenges that physicians face is the poor adherence to treatment.

Methods: We designed a cross-sectional study in patients with Parkinson’s disease in which demographic and clinical data were collected. The following instruments were used: Hoehn & Yahr scale, Movement Disorder Society – Unified Parkinson’s Disease Rating Scale (MDS-UPDRS), Parkinson’s disease questionnaire (PDQ-8), the European Quality of Life Questionnaire 5 level version (EQ-5D), the Spanish Chronic Disease Self-Efficacy scale, Morisky-Green test, Schwab & England Activities of Daily Living Scale (S&E) and a brief questionnaire about patient’s treatment.

Results: A total of 86 patients were included. We found a mean age of 61.98 ± 12.07 years & 62 (72.1% of the sample) were male. 46.5% & 32.6% had a Hoehn & Yahr score of 1 and 2, respectively. 52.3% had a mild MDS-UPDRS. 38.4% & 34.2% of patients were treated with monotherapy and combined therapy, respectively.

A statistically significant difference was found in patients with good self-efficacy (p=0.015) and with good quality of life (p=0.001). We found no significant relationship between sex, age, mental status, Parkinson’s disease duration, number of drugs taken, Hoehn & Yahr score nor the level of education. For non-parametric variables, we found a relationship with the S&E scale (p=0.028) (see Table 1).

Conclusions: These findings suggest that a good quality of life and a good self-efficacy predict a good adherence to treatment in patients with Parkinson’s disease.
