Application of PKG System for Objective Measurement and Early Identification in Parkinson’s Disease

Objective: This study aimed to assess the correlations of the PKG data with clinical scores of bradykinesia, rigidity and tremor.

Background: The treatment of levodopa for Parkinson’s disease (PD) is effective, but 50% of patients may develop “wearing off”, motor fluctuations and dyskinesia after 5 years. Home diary,Unified Parkinson Disease Rating Scale(UPDRS) is used for clinical evaluation, which can‘t monitor symptoms continuously. The Parkinson’s KinetiGraph (PKG) is the wearable sensor system to receive clearance from the FDA to provide objective ratings of Parkinson’s motor features continuously, including bradykinesia, dyskinesia, and fluctuations.This is the first study to separately analyze the correlation between PKG data and the scores of different motor symptoms in UPDRS scale.

Method: An observational study of 100 PD patients with a PKG data logger ≥ 6 days was performed during routine daily living activities.The PKG consists of a series of algorithms that provide data points every 2 minutes (recorded between 6 am and 10 pm), then these charts and scores synthesize this data into clinically useful formats such as the percent time tremor(PTT), dyskinesia score (DKS) and bradykinesia score (BKS), All patients completed  scale evaluations in good condition after taking the medicine. Finally collected PKG data and UPDRS scores were analyzed, including UPDRS Ⅲ total scores,UPDRS Ⅲ-bradykinesia scores (item 23-26,31 ), and UPDRS Ⅲ-rigidity scores(item 22), UPDRS Ⅲ-tremor scores (item 20,21) .

Results: It showed that there was significant correlation between BKS and the scale scores of UPDRSⅢ,  including UPDRS Ⅲ total scores,UPDRS Ⅲ-bradykinesia scores, and UPDRS Ⅲ-rigidity scores(Pearson’s correlations coefficients: 0.479-0.588, p<0.05) and the correlation between between PTT scores and UPDRS Ⅲ-tremor scores, UPDRS Ⅱ-tremor scores was significant(r=0.223, r=0.343, p<0.05). Through subgroup analysis, we found that in early stage( H-Y stage:1-2) or early disease course(less than 3 years), BKS and UPDRS Ⅲ scores were better correlated( r2=0.465, r2=0.441, p<0.05).

Conclusion: The study shows that thers is significant correlation between PKG data with  bradykinesia, rigidity and tremor scores from UPDRS III,especially in early stage or early disease course.And it supports that the feasibility of PKG to detect abnormal movements in patients with PD, and it is suitable for early detection and timely treatment.
