Clinical and paraclinical aspects of extrapyramidal disorders in patients with cerebrovascular pathology

Objective: To study of clinical and paraclinical aspects of extrapyramidal disorders in patients with cerebrovascular pathology

Background: A considerable proportion of patients with chronic cerebral ischemia (CIM) or those who have suffered a cerebral stroke may develop varying degrees of extrapyramidal motor disorders.

Method: A total of 62 patients, including 28 men and 34 women. The average age of the patients was 67.4±10.1 years. The studied patients were divided into two groups: 38 patients with extrapyramidal disorders associated with CIM and 24 patients with extrapyramidal disorders following a cerebral stroke. Clinical assessment included neurological examination, ultrasound Dopplerography (USD) for cerebral hemodynamics, and MRI for structural brain analysis.

Results: In the first group, vascular parkinsonism was identified in 20 patients (52.6%), while hyperkinetic extrapyramidal disorders were found in 18 patients (47.4%). Among those with hyperkinetic disorders, 6 patients exhibited hemitremor, 3 had facial blepharospasm, 2 presented with hemichorea, 5 suffered from restless legs syndrome, and 2 were diagnosed with dystonia. In the second group, vascular parkinsonism was detected in 10 patients (41.7%), while hyperkinetic extrapyramidal disorders were observed in 14 patients (58.3%). Among these, 4 cases of hemitremor, 2 cases of chorea, 1 case of blepharospasm, 2 cases of facial hemispasm, 3 cases of tics, and 2 cases of restless legs syndrome were recorded. MRI analysis showed a predominant location of leukoareosis outside the subcortical nodes in CIM patients (73.6%), while stroke patients exhibited leukoareosis primarily in the basal ganglia (70.8%).

Conclusion: Extrapyramidal disorders can arise in both CIM and post-stroke patients, with varying manifestations of vascular parkinsonism and hyperkinetic disorders.
