Objective: To systematically identify and evaluate the facilitators and barriers influencing the implementation of exercise programs in Parkinson’s disease (PD) rehabilitation using the Consolidated Framework for Implementation Research (CFIR).
Background: Exercise is widely recognized as an essential component of PD rehabilitation, improving motor function, balance, and quality of life. However, challenges persist in translating evidence-based exercise programs into routine clinical practice. A structured analysis of implementation barriers and facilitators is necessary to optimize exercise adherence and accessibility.
Method: A systematic review was conducted across Cochrane, PubMed, Scopus, and Web of Science databases. Studies assessing exercise program implementation in PD rehabilitation were included, with data extraction based on CFIR domains: intervention characteristics, outer setting, inner setting, characteristics of individuals, and implementation process. Thematic synthesis was performed to categorize key facilitators and barriers.
Results: A total of 98 studies met inclusion criteria. Key facilitators included strong provider endorsement, structured supervision, and integration with telemedicine platforms. Barriers encompassed logistical constraints (e.g., travel difficulties), variability in patient motivation, and inconsistent reimbursement policies. Programs with community engagement and hybrid delivery models demonstrated the highest adherence rates. Additionally, digital interventions improved accessibility but required technological literacy support.
Conclusion: Successful implementation of PD exercise programs requires a multi-faceted approach addressing patient, provider, and systemic factors. Integrating tele-exercise, financial incentives, and personalized coaching may enhance program sustainability. Future research should focus on refining intervention strategies to improve long-term adherence and health outcomes.
