Sleep related disorders in subtypes of patients with Parkinson’s disease in the Tomsk region, Russia

Objective: To describe sleep related disorders (SRD) in Tremor Dominant (TD) and Akinetic Rigid (AR) patients with Parkinson’s disease in (PwPD) subtypes evaluated at Movement Disorders Center, Tomsk, Russia.

Background: Currently no data regards SRD in PwPD with TD and AR subtypes in Tomsk region. Non motor symptoms especially SRD add significant disease burden in PwPD.

Methods: Tomsk electronic movement disorders database includes 796 PwPD. Clinical assessments included demography, disease parameters (PD duration, PD type, drugs number, SRD duration, excessive daytime sleepiness (EDS)), UPDRS, Hoehn and Yahr Scale (H&Y), Hospital Anxiety and Depression Scale, Epworth Sleepiness Scale, PD sleep score (PDSS), Montreal Cognitive Assessment (MoCA-test), PD Questionnaire-39 (PDQ-39).

Results: Two PwPD groups were examined (homogeneous of gender, age, disease duration, H&Y stage: mean age: 66.8±6.3 years; mean PD duration: 6.8±4.6 years; H&Y 1–4): I- 88 TD, II- 69 AR. 86.6% PwPD reported SRD: 94.3% in TD group, 76.8%– AR. REM-sleep behavior disorder (RBD) was described in 43.3% PwPD (48.9% TD, 36.2% AR). 22.9% PwPD with RBD injure themselves (27.3% TD, 17.4% AR) or their bed-partner 30.5% (38.6% TD, 20.3% AR). SRD were most prominent in PwPD at advanced stage (p<0.05). 48.4% PwPD reported EDS (52.3% TD, 43.5% AR). In PwPD with EDS in both groups lower life quality was found compared with PwPD without sleepiness by following PDQ-39 indexes: mobility, activity, cognitive (p<0.05). The overall total PwPD PDSS score was 106.68±23.39 (range: 30-144). According PDSS 55 PwPD (35.0%) had moderate to severe SRD (44.3% TD, 20.3% AR). Significant differences were in disease duration, H&Y, UPDRS section subscores between groups of PwPD either with or without SRD (p<0.05). SRD correlated with daily life activity, mobility, tremor intensity (p<0.001).

Conclusions: 86.6% of PwPD had SRD prevailed in TD subtype. This area needs further research. Normalization of sleep disorders can improve the life quality in PwPD.
