Objective: To compare the volumetric changes between patients at prodromal stage of Lewy body disese and individuals with dementia with Lewy Bodies.
Background: Volumetric magnetic resonance imaging (MRI) has been used over past decade as a method to help with the clinical diagnosis of Alzheimer’s dementia. We investigated if that technique would be useful to differentiate between individuals with Dementia with Lewy Bodies (DLB) from prodromal Lewy body disease (MCI-LBD) patients. This was a retrospective study planned to compare the volumetric MRI values between these two group of patients.
Methods: The investigation involved 11 patients with diagnosis of DLB, according to the third report of the DLB Consortium and 4 patients with prodromal Lewy body disease (MCI-LBD), defined as MCI with suggestive features of Lewy body pathology ( possible RBD and/or Parkinsonism), without prior diagnosis of any Parkinsonian syndrome. All the subjects were submitted to a standard protocol, including volumetric MRI evaluations. We specifically compared volumetric percentile in nineteen categories.
Results: Total ventricle volume and total white matter in DLB patients were in lower percentile compared to pro LBD but the difference was not statistically significance. The results indicated that four regions of interest (Medial temporal, inferior parietal, .fusiform and primary sensory) were significantly different (p < .05) in DLB patients compared to MCI-LBD subjects. We did find that MCI-LBD patients (vs. DLB patients) had more voluminous fusiform gyri (65th vs. 32nd, p=0.04), Middle Temporal (59th vs. 31st, p=0.048), Inferior Parietal (63th vs. 40th, p=0.048) and Primary Sensory (63rd vs. 43th, p=0.03) based on average percentile.
Conclusions: The results suggest that measures of these four regions maybe useful to identify DLB patients and MCI-LBD subjects from normal elderly individuals. The atrophy of fusiform gyrus raise the question if the face recognition can have more clinical application in the future for detecting prodromal stage of the Lewy body diseases.
