Correlates of gait impairment in parkinson’s disease

Objective: Assess gait dysfunction in patients with PD and its correlation with clinical (motor and non-motor symptoms) and radiological findings.

Background: Gait impairment is one of the most disabling symptoms in Parkinson’s disease (PD). Focused gait assessment in different PD motor subtypes could promote for better understanding of its multifactorial origin and correlation with motor and non-motor symptoms.

Method: Patients with PD were assessment by MDS UPDRS, Gait Assessment by Berg Balance scale (BBS), 10 Meter Walk Test (10MWT), time up & go test (TUG) and new freezing of gait questionnaire (NFOG-Q) during OFF and ON. Other assessments included non-motor assessment using MMSE for cognitive assessment, nonmotor symptoms scale (NMSS), Beck depression inventory (BDI), Parkinson’s disease questionnaire (PDQ-39), in addition to MRI Brain.

Results: 91 patients with PD were recruited [55 patients tremor dominant (TD), 25 patients with postural instability and gait disorder (PIGD) , and 11 patients with indeterminat motor subtypes].The mean age was (55.1 ± 9.3) and mean age of onset and duration of illness were (48.7±9.6 and 6.3 ± 3.9 respectively). MDS -UPDRS total and part III ‘OFF state’ were (99.8 ±37.9, 57.4 ±21.8, respectively) and TD showed a significant better scoring than other subtypes.The mean H & Y was 2.98 ±0.95 and the mean LEDD was (767.9 ± 449.9). Gait impairment was significantly impaired in PIGD and the indeterminat subtypes using NFOG-Q and TUG ‘OFF’ (P<0.001), BBS and 10 MWT ‘OFF’ (p=0.001). There was significant correlation between (duration of illness, LEDD, MMSE, NMSS, BDI, PDQ-39) and (10 MWT, NFOG-Q, BBS, MDS-UPDRS total and part III ‘OFF’ ).TUG ‘OFF’ was also correlated with NMSS, BDI and PDQ-39.

Conclusion: Gait impairment in PD negatively impacts QoL and correlated with cognitive and non-motor symptoms. Comprehensive approach is needed to manage properly and guard against its sequalae.
