Endophenotyping adult-onset isolated cervical dystonia: Does period of onset of mood disorder matter?

Objective:
We aim to characterize clinical and demographic features pertaining to the time of onset of mood disorder in CD patients.

Background: Mood disorder frequently occurs in cervical dystonia (CD) which is the most prevalent form of adult-onset isolated focal dystonias.

Method: We retrospectively collected demographic data and assessed patients using the CD Impact Profile-58 (CDIP-58), EuroQol-Utility Values (EQoL-UV), Beck’s Anxiety & Depression Inventories (BAI & BDI-II) and Toronto Western Spasmodic Torticollis Scale-2 (TWSTRS-2). Given the non-gaussian nature of our data, we mostly analyzed using non-parametric tests.

Results:
In 201 CD patients (136 women, 65 men), mean age was 61.46(±12.71). 79/201(39%) patients had mood disorder (anxiety, depression or both), 46/201(23%) were diagnosed prior to the onset of CD and 33/201(16%) after the onset of CD. In addition to having worse BAI and BDI-II scores, patients with mood disorder had an earlier age at onset of CD (p=0.0393), worse quality of life(CDIP-58 p=0.002; EQoL-UV p<0.0001) and worse TWSRTRS-disability scores(p=0.0067) compared to those without. The 33/201(16%) with mood disorder after the diagnosis of CD had worse BAI scores than patients with mood disorder prior to the onset of CD (p=0.0104).

Conclusion:
CD patients with any mood disorder at any time have worse HrQoL and report more disability than patients without but patients who developed mood disorder following a diagnosis of CD reported the worst quality of life of all the groups and exhibit more anxiety than those with mood disorder prior to the onset of their CD. More attention in relation to mood should be given to this group in clinics.
