Objective: To investigate expression of the PD-related pattern in 18F-FDG PET scans of iRBD patients longitudinally.
Background: The Parkinson’s disease-related pattern (PDRP) might serve as a potential marker for disease progression in PD and tis prodrome: isolated REM sleep behavior disorder (iRBD).
Method: Thirteen iRBD patients underwent clinical assessments and 18F-FDG PET brain scans at baseline, follow-up 1 (FU1, 4 years) and follow-up 2 (FU2, 8 years). Four patients progressed to PD, and one to dementia with Lewy bodies. Three patients had subthreshold parkinsonism. Five were clinically stable. We computed PDRP z-scores and conducted longitudinal analyses.
Results: Only converters exhibited a significant increase in PDRP z-scores from BL to FU1 and FU1 to FU2. We found an interaction effect between conversion status and time on PDRP z-scores.
Conclusion: PDRP expression increases linearly near phenoconversion. Repeated 18F-FDG PET brain scans may be used to predict short-term conversion in iRBD patients.
