Prevalence of Arm Tremor Associated With Dystonia (Arm-TAWD) in patients with Adult-onset Isolated Cervical Dystonia in an Irish population.

Objective: The aim of the study was to determine whether Arm-TWAD affects HrQoL in patients with adult-onset isolated cervical dystonia (AOIFD-CD).

Background: TAWD is tremor in Dystonia appearing in locations where there is no dystonia. In AOIFD-CD patients, the arm is the commonest location for TAWD.

Method:
We prospectively examined all AOIFD-CD patients in our dystonia database for Arm-TAWD and collected demographic information. We also assessed all AOIFD-CD patients using the CD Impact Profile-58 (CDIP-58), EuroQol-Utility Values (EQoL-UV), Beck’s Anxiety & Depression Inventories (BAI & BDI-II) and Toronto Western Spasmodic Torticollis Scale-2 (TWSTRS-2). Non-parametric test were used for analyses.

Results:
160 patients were examined, 108 women and 52 men. 27/160(17%) of all patients had arm tremor (Arm-TAWD) and 133/160(83%) had no arm tremor (No-Arm-TAWD). This was more prevalent in women 20/108(19%) than men 7/52(13%). All women had poorer HrQoL than all men (CDIP-58 p=0.0031 and EQoL-UV p=0.0005). In the No-Arm-TAWD group, women also had worse HrQoL (CDIP-58 p=0.0042 and EQoL-UV p=0.0002), BAI (p=0.0445), pain(p=0.0386) and disability (p=0.0117) than all men; however, there was no significant difference between the Arm-TAWD and the No-Arm-TWAD group in CDIP-58, EQoL-UV, BAI, BDI-II, Pain, Disability, Severity, Age at onset of dystonia and Duration of CD.

Conclusion: Arm-TAWD is more prevalent in women with AOIFD-CD than men. There were similarities between the Arm-TAWD and No-Arm-TAWD groups in measures of HrQoL, symptoms of anxiety and depression, pain, disability from dystonia and severity of dystonia.
