Meeting: 2018 International Congress

Keywords: Cognitive dysfunction, Functional magnetic resonance imaging(fMRI), Parkinsonism

Session Information

Date: Monday, October 8, 2018

Session Title: Parkinson's Disease: Cognition

Session Time: 1:15pm-2:45pm

Location: Hall 3FG

Objective: To evaluate the patterns and magnitude of functional connectivity in resting-state brain networks in patients with Parkinson’s disease and cognitive impairment by pooling data from eligible studies.

Background: Cognitive impairment is a common non-motor symptom in Parkinson’s disease. So far, the underlying pathophysiology remains unclear. Several alterations in functional network connectivity have been described, predominantly in the default mode network and fronto-parietal network. However, the reported resting-state connectivity patterns vary greatly among studies and individual studies often lack sufficient power.

Methods: We searched PubMed, the Cochrane Library, MEDLINE, Embase and PsycINFO to identify functional MRI studies in patients with Parkinson’s disease and cognitive impairment. A voxel-based meta-analysis combined with quality statistics was performed, using the anisotropic effect-size version of the signed differential mapping method (AES-SDM).

Results: Fourteen studies with cognitively impaired Parkinson’s disease patients were included, consisting of 273 Parkinson’s disease patients without cognitive impairment, 168 Parkinson’s disease patients with mild cognitive impairment, and 59 patients with Parkinson’s disease dementia, as well as 269 healthy controls. Patients with Parkinson’s disease and cognitive impairment predominantly showed a reduced connectivity in brain regions that are part of the default mode network and auditory network.

Conclusions: Cognitive impairment in Parkinson’s disease is associated with reduced connectivity in networks relevant to cognition, specifically the default mode network and auditory network. Functional connectivity alterations may serve as a biomarker for cognitive impairment in Parkinson’s disease and contribute in unravelling its pathophysiology. Abstract has been presented before at one other meeting: NVN Wetenschapsdagen, 9-10 November 2017, The Netherlands. (Poster presentation)
