Short term effects of bilateral STN DBS on psychiatric profile in Parkinson’s disease

Objective: We intend to look into the effects of bilateral STN DBS on Psychiatric profile of Parkinson’s patients.

Background: Parkinson’s disease is a well-known degenerative neuropsychiatric disorder and deep brain stimulation offers significant improvement in motor symptoms. Various side effects are reported in patients undergoing DBS ranging from cognition to mood changes and much of the literature is from western population.

Methods: All Parkinson’s disease patients undergoing bilateral STN DBS in the year 2016 were preoperatively evaluated for any Psychiatric illness with Mini International Neuropsychiatric interview MINI (version 7.0) and repeated six weeks after the surgery.

Results: A total of 27 patients who underwent bilateral STN DBS were evaluated pre-op and 6 weeks post op. Male to female ratio was 1.25:1 with mean age of 57.9 years and mean duration of disease was 9.8 yrs. Pre operatively five patients has depression, 4 patients had psychosis, 2 patients had suicidal thoughts, 2 panic disorder, 1 patient had agoraphobia, 1 pt had social phobia and one patient had generalized anxiety disorder. At six weeks follow up 3 pts improved with depression, one pt with panic disorder improved one pt with generalized anxiety improved and 2 patients had acute worsening of psychosis in the immediate post op period which later improved to pre-op state. Two of three patients who had improvement in depression had their syndopa dose decreased, and one pt with improvement in both panic disorder and generalized anxiety also had decreased dose of syndopa.

Conclusions: Short term effects of bilateral STN-DBS on Psychiatric profile in Parkinson’s patients appear to be beneficial, much of these are due to decrease in anti-parkinson’s medication which is an indirect effect of the surgery. However, long term effects of these findings need to be confirmed.
