Objective: 1) To study prevalence and clinical characteristics of non-motor symptoms (NMS) in idiopathic parkinson disease (IPD) and atypical Parkinsonism (APD) as compared to age and sex matched controls 2) To compare the prevalence of NMS as well as NMS burden (symptoms frequency × severity) between typical and atypical Parkinsonism.

Background: Parkinsonian disorders are one of the most prevalent neurodegenerative disorders. Both motor as well as non-motor symptoms of parkinsonism have significant effect on quality of life. Relatively little is known on the epidemiology and clinical characteristics of non-motor symptoms (NMS) in patients with typical and atypical parkinsonism in Indian population. This is a cross-sectional, single center, open label, one point in time evaluation study conducted from November 2016- April 2017, to assess the prevalence of NMS in a large cohort of Indian patients affected by various parkinsonian syndromes (typical as well as atypical) will guide for the management of these symptoms and thereby improve quality of life.

Methods: Patients fulfilling the standard diagnostic criteria for parkinsonian disorders were included in study. Data was collected as per pre-defined proforma and entered in Excel. Controls were age and sex matched accompanying persons of other patients. Data analysis was done with SPSS (Statistical Package for the Social Sciences) Software version 21. Quantitative data is presented with the help of Mean, Standard deviation, Median and IQR (interquartile range), comparison among three study group was done with the help of One-way Anova test. P ≤ 0.05 was taken as significant.

Results: As compared to control group maximum number of non-motor symptoms was significantly high in both IPD and APD patients. In IPD patients, commonest non-motor symptom reported was sexual dysfunction (81%), followed by fatigue (70.45%) and constipation (68.18%). In APD patients, commonest non-motor symptom reported was also sexual dysfunction (85%), followed by urinary urgency (72.5%) and fatigue, lack of motivation and nocturia in 67.5%. Symptoms of restless legs was significantly more in IPD group (p ≤ 0.05). However, association of the most other non-motor symptoms was significantly more in APD patients (p ≤ 0.05). Mean of total NMS (non-motor symptoms) score was higher significantly in APD patients (p ≤ 0.05).

Conclusions: Prevalence of non-motor symptoms as well as non-motor symptom burden scores are highest (p < 0.05) for all domains in atypical parkinsonism as compared to Idiopathic parkinsonism patients and controls.
