The correlation between dietary fiber, fluid intake and constipation in patients with Parkinson’s disease

Objective: To clarify the correlation between high dietary fiber, fluid intake and constipation in Parkinson’s disease (PD) patients.

Background: Constipation is one of the complications of PD, and is sometimes related to the ileus. Previous studies reported that intake of a large amount of dietary fiber improved constipation in the general population. The effect of large amounts of dietary fiber in PD patients is not currently known.

Methods: We evaluated out-patients with PD using a cross-sectional study. We estimated daily dietary fiber (total, soluble and insoluble) and fluid intake for three days (two week days and one weekend day) using food diaries and pictures, and divided patients into three groups (tertiles); low, moderate and high dietary fiber or fluid intake. Constipation was evaluated using ROME III criteria for functional constipation, Bristol Stool Form Scale (BSFS) and Constipation Scoring System (CCS).

Results: 42 subjects (13 males, mean age 71.8 (SD 9.4) yr., disease duration 7.4 (SD 4.1) yr., HY stage 2.2 (SD 0.6), MMSE 26.8 (SD 4.0)) were included. The mean total, soluble and insoluble daily dietary fiber intakes were 18.2 (SD 5.5) g, 3.9 (SD 1.0) g and 13.0 (3.8) g, respectively. The mean daily fluid intake was 1450.2 (SD 312.6) ml. 27 patients were ROME III-positive, mean BSFS was 3.2 (SD 1.2) and mean CSS was 6.1 (SD 4.3). ROME III-positive male patients (Pearson’s chi-square test; p = 0.017) and patients with higher CSS scores (Kruskal-Wallis test; p = 0.016) consumed high soluble dietary fiber. There was no correlation between total dietary fiber intake, fluid intake and constipation in patients with PD. Laxatives and past surgery of digestive tract did not influence on the results.

Conclusions: PD patients with constipation showed a tendency to take larger amounts of soluble dietary fiber.
