Transcranial imaging of substantia nigra hyperechogenicity in a Chinese cohort of Parkinson’s disease

Objective: In this research we set out to study the diagnostic value of TCS in Parkinson’s disease.

Background: An increased echogenicity of the substantia nigra (SN) in patients with Parkinson’s disease (PD) was demonstrated by transcranial ultrasound (TCS).

Methods: A total of 222 subjects including 139 patients with PD,61 cases in the controls,9 patients with PD plus syndrome and 19 ET, were recruited from the movement disorders clinic at the Department of Neurology, Tongji Hospital affiliated to Huazhong University of Science and Technology, wuhan, China. All subjects were diagnosed by neurological physician according to 2016 MDS diagnostic criteria. Transcranial sonographic evaluations of the SN were performed in all subjects, motor and non-motor symptoms were thoroughly assessed by a series of rating scales in PD patients.

Results: 1. The mean size of the SN+ in PD was greater as compared to NPD subjects (p < 0.005). Sensitivity was 79.9%, and specificity was 72.7%. 2.Compared with contrals, the SN echogenicity of ET subjects is greater (P<0.005), but not meet the diagnostic criteria of PD. 3.There is no correlation between SN echogenicity and clinic features including time of onset, disease duration. subgroup. disease severity. 4.In the control subjects, males had larger SN echogenic areas. Compared to females (12.94 ± 13.03 vs. 2.66 ± 5.37mm2, p < 0.001). However, there is no difference in the PD.

Conclusions: TCS is an effective diagnostic tool for detecting PD in Chinese. However, it does not reflect disease characteristics of parkinsonism.
