Utility and safety of same day discharge after Deep Brain Stimulator Electrode Implantation: a feasibility study

Objective: The purpose of this study is to determine the demographics and characteristics of patients diagnosed with Parkinson’s Disease or Essential Tremor who can be discharged home safely post deep brain stimulator electrode implantation surgery.

Background: The role of Deep Brain Stimulation (DBS) surgery in managing symptoms of Parkinson’s disease has increased over the years. Historically, in an effort to reduce the most neurologically significant risk of intracranial hemorrhage, these patients were observed in an ICU setting with hourly blood pressure monitoring and treatment with IV antihypertensives as needed. This caused significant utilization of healthcare resources when only a small subset of patients required treatment with antihypertensive agents.

Method: This is an observational study using multivariate analysis and logistic regression. A detailed review of patient characteristics prior to DBS surgery is used to determine the risk factors that predict discharging patients home safely post-surgery. Data collected include age, sex, comorbidities, past surgeries, HTN hx, and medication. Patients diagnosed with Parkinson’s disease or essential tremor who underwent placement of unilateral or bilateral DBS electrodes between 2022-2023, are included in the study. Patients were evaluated primarily based on their blood pressure control before, during and after surgery to determine safe same discharge home where as overnight hospital stay.

Results: Of the cohort of patients that underwent DBS surgery in 2022 and 2023, six did not have SBP > 140 during their hospitalization were discharged home without any noted neurological sequelae or readmission to the hospital.

Conclusion: The result of this study suggests that we can identify patient characteristics that will ensure safe same day discharge post DBS Electrode and the potential healthcare resource savings. Same home discharge post DBS surgery will potentially help decrease healthcare costs associated with the surgery and open up hospital beds for patients who are in need of them.
