The importance of vascular anomalies and endothelin-1 factor in the development of vascular parkinsonism

Objective: To study the relationship between vascular endothelium and vascular anomalies in the development of vascular parkinsonism.

Background: Although vascular parkinsonism (VP) is relatively rare, it is not observed in all patients with impaired cerebral blood circulation. Of course, it is closely related to the vascular endothelium and its integrity.

Method: 46 patients (23 men and 23 women) with VP were examined. The mean age of the patients was 62.8±5.4 years. The mean duration of the disease was 6.5±4.3 years. Endothelin-1 factor was examined by IFA. Vascular anomalies were detected by MRI-angiography. The control group consisted of 20 patients without VP. Statistical analysis found p≤0.001 to be reliable.

Results: The serum endothelin-1 level in patients with VP was on average 17.8±4.8 5.9 pg/ml, while in the control group it was 8.1±3.3 pg/ml, a significant difference r≤0.001. When examining patients with high levels of endothelin-1 in the main group, aneurysms, hypoplasias and aplasias were detected in the vessels in 29 patients (63%), and this was most often observed in the middle cerebral artery (a. cerebri media). In the main group of patients with vascular anomalies, the average serum endothelin-1 level was 19.6±5.8 5.9 pg/ml, and in patients without anomalies it was 16.6±4.4 pg/ml, r≤0.001.

Conclusion: In the development of vascular parkinsonism, not only changes in the vascular endothelium, but also vascular anomalies are important. Elevated levels of endothelin-1 factor and the combination of aneurysms, hypoplasias, and aplasias are risk factors for the development of vascular parkinsonism.
