RBD increase non-motor symptom in Parkinson’s disease

Objective: To investigate the relationship between the RBD and other non-motor symptoms in Parkinson’s disase.

Background: Rapid eye movement sleep behavior disorder (RBD) is frequent in Parkinson’s disease (PD) and may be a presymptom of PD, otherwise, some studies focus on the special characteristics of RBD+PD.

Methods: We examined 150 PD patients who were enrolled from our hospital from January 2013 to August 2014. We assessed for the presence of with the REM Sleep Behavior Disorder Screening Questionnaire (RBDSQ). All patients were assessed for motor subtypes with the Unified PD Rating Scale (UPDRS) III in the “on” state, and compared the sub-scale scores representing tremor, rigidity, appendicular and axial signs. They were also assessed by the Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale (HAMD), Mini-Mental State Examination (MMSE), and Parkinson’s disease Sleep Scale (PDSS).

Results: We excluded other parkinsonism disorders and 141 PD patients were entered the final study. Thirty (21.28%) PD patients had probable RBD (pRBD) diagnosed with a RBDSQ score of 6 or above. There were no significant differences for age of PD onset, PD duration, gender, smoking status, alcohol or coffee use, presence of hyposmia or freezing, UPDRS III, and H-Y stages between the pRBD+ and pRBD- groups. We found that pRBD+ PD patients had lower MMSE scores, higher PDSS scores, and more prominent proportion in anxiety, depression, constipation, hallucination and greater prevalence of orthostatic hypotension.

Conclusions: In age and gender matched conditions, pRBD+ PD patients exhibited greater changes in non-motor symptoms. However, there was no increase in motor deficits.
