Objective: To assess whether global or regional brain structural abnormalities are associated performance of activities of daily livings (ADL) in patients with Parkinson’s disease (PD).
Background: The contribution of bran abnormalities of PD patients to their function status remains uncertain.
Method: Forty-six patients with PD from a movement disorder clinic were recruited prospectively. Motor impairment and disability were assessed using the Hoehn and Yahr (H-Y) scale and the Unified PD Rating Scale, part III (UPDRS-III). Cognitive status was evaluated with Montreal cognitive assessment (MOCA). The performance of ADL was measured by the sum score of Physical Self-Maintenance Scale (PSMS) and Lawton Instrumental ADL scale. Brain magnetic resonance imaging (MRI) examinations were performed to assess the severity of white matter hyperintensities, and the presence of silent lacunes, medial temporal lobe atrophy (MTLA). Indexes of global brain atrophy, including the relative gray matter volume (RGM), relative white matter volume (RWM) and average cortical thickness of the whole brain were quantitatively measured by voxel-based morphometry (VBM). The crude associations of variables with ADL were examined using Pearson or Spearman correlation analyses. Multiple linear regression analysis was performed to find the associated factors of ADL in PD patients.
Results: ADL score (higher indicating poorer performance) negatively correlated with RWM (r = -0.41, P=0.004) and RGM (r = -0.43, P=0.003), but not with average cortical thickness (r = -0.16, P=0.29). With ADL score as the dependent variable in a linear regression model, H-Y stage and RWM were significant correlates of ADL score adjusted for age and MOCA score , accounting for 51% of the variance of it. RGM was not a significant correlate for ADL score after adjusted for age and MOCA.
Conclusion: Cerebral white matter atrophy may be associated with the performance of ADL in patients with PD.
