Objective: To evaluate the efficiency of DBS STN in PD patients in Republican center of neurology and neurosurgery
Background: Deep brain stimulation (DBS) of the subthalamic nucleus (STN) is considered safe and effective in advanced Parkinson’s disease (PD), but depends on the duration of neuromodulation [1]. DBS STN is effective for correct motor symptoms, but the patients’ quality of life (QoL) depends on other components as well [2].
Method: 53 patients with DBS STN, 34 m and 19 f, 55,6±7,3 y, PD duration 10,0[9,0;13,0]. The postoperative follow-up period was 5 years.
To assess the motor symptoms used UPDRS, Postural Instability and Gait Disorder – PIGD (consists of score 13,14,15,29,30 items of UPDRS), to assess the QoL – PDQ-39.
Results: The score by III p UPDRS in off-med period decreased on DBS STN ANOVA χ2 = 8,35, p = ,00001 (Fig 1). In 5 y – 19,0 (95%CI 10,4;25,7) (W, p= ,008). The score by III p UPDRS in on-med decreased too on DBS STN in 5 y, but not as significant (W, p=0,02).
PIGD in off-med period decreased on DBS STN ANOVA χ2 = 2,47, p = ,04 (Fig 2), but didn’t in on-med ANOVA χ2 = 1,81, p = ,13.
The dynamics of PDQ-39 scores and sub-scores during the DBS STN are summarized in the table (Fig. 3).
Conclusion: DBS STN effective to correct basic PD motor symptoms and PIGD for 5 y follow-up period. QoL improved significant after 1 year, but by year 5 the improvement was maintained for individual subgroups – BD and S. TS of PDQ-39 increased by 5 y. QoL in long-term DBS STN depends on non-motor symptoms of PD.
