Substantia nigra hyperechogenicity in Mexican patients with Parkinson’s disease

Objective: To determine the prevalence of increased echogenicity in the substantia nigra in patients diagnosed with idiopathic Parkinson’s disease at the University Hospital "Dr. José Eleuterio González ".

Background: Introduction: Parkinson’s disease is the second most common progressive neurodegenerative disease of adult onset and the second movement disorder. Substantia nigra (SN) hyperechogenicity is a biomarker for Parkinson’s disease (PD). This is a SN hyperechogenicity measure by transcranial ultrasound in a cohort of patients with PD in North of Mexico.

Methods: Methods: This is a cross-sectional study, SN hyperechogenicity was evaluate in 35 patients with diagnosis of Parkinson’s disease, from the Parkinson´s Disease and Movement Disorder Clinic University Hospital "Dr. José Eleuterio González ". Midbrain of each patient were visualized by transcranial ultrasound following and standardized protocol.

Results: The substantia nigra was assessable in 92% of all participants, in 2 (5.6%) the study was negative. We confirmed that SN echogenicity is significantly increased in PD patients, 30 patients (85.7%) were found an increased echogenicity in the topography of the substantia nigra (larger of bilateral measurement) of the midbrain level ≥ 0.19cm2, there were not correlated with age. We concluded a study as positive only with having observed the alteration of the substantia nigra unilaterally in the midbrain peduncle.

Conclusions: This study indicates the merit of manual measurement of SN hyperechogenicity by transcranial sonography in PD diagnosis in Mexican patients and demonstrates that transcranial sonography of the substantia nigra is a relevant and useful diagnostic tool for patients with PD. As best of we know this first study that concluded a study as positive only with having observed the alteration of the substantia nigra unilaterally in the midbrain peduncle, although the contralateral peduncle for some reason could not be displayed.
