Vascular, inflammatory and metabolic risk factors in relation to dementia in Parkinson’s disease patients with type 2 diabetes mellitus

Objective: Identifying the risk factors of dementia in patients with both Parkinson’s disease (PD) and type 2 diabetes mellitus (DM) (PD-DM) is clinically relevant and has been rarely investigated.

Background: Dementia in patients with comorbid Parkinson’s disease (PD) and type 2 diabetes mellitus (DM) seriously accelerates the neurodegenerative process. Identifying the risk factors of dementia in patients with both PD and DM (PD-DM) is clinically relevant and has been rarely investigated.

Method: We performed a large retrospective cohort study (January 2013 to May 2019) and compared 215 PD-DM patients (including 31 PD-DM with dementia, PD-DMD), 341 PD patients without DM (including 31 PD with dementia, PDD) and 372 DM patients without PD (including 35 DM with dementia, DMD). Serum vascular, inflammatory, metabolic biomarkers, magnetic resonance imaging (MRI), medication and clinical variables were analyzed. Pearson’s χ2 tests, Mann-Whitney U test, Kruskal–Wallis one-way analysis of variance or Dunn-Bonferroni test, and binary logistic regression were used to investigate potential risk factors for PD-DMD. Receiver operating characteristic (ROC) analysis was used to find reliable prognostic clinical variables for PD-DMD.

Conclusion: We identified clinical factors including fibrinogen, LDL, SAE, and WMLs, as most significant risk factors for PD-DMD patients; they were more prevalently associated with PD-DMD than PDD and might have prognostic and treatment implications.
