What strategies do Parkinson’s disease patients use for getting out of bed in the early morning? Preliminary results from home video analysis of 22 patients.

Objective: To classify PD patients’ getting out of bed strategies using home video analysis.

Background: Getting out of bed represents a major difficulty of early morning akinesia in Parkinson’s disease (PD) patients. However, few studies have investigated the strategies patients employ to overcome this problem.

Methods: PD patients with subjective complaints of early morning akinesia were videotaped in their bedroom environment, and assessed by two independent neurologists. Getting out of bed movements were classified into: 1) head and trunk, 2) far upper limb, 3) near upper limb, and 4) lower limbs, with time taken to transfer from horizontal position to standing also measured.

Results: 22 PD patients (16 M, 6 F; a mean age 63.1 + 11.3 years; HY stage 2.9 + 0.83) were included [table1]. The majority of patients (72.7%) could get out of bed independently, but their strategies differed from healthy adults. Common movements for head-trunk were: come to sit (27.8%) and lateral roll (27.8%); for far upper limb: being push (55.6%); for near upper limb: being push (38.9%); and for lower limbs: step off (27.8%) [table2]. Remaining 6 patients (27.3%) were helped by caregivers or used assisted devices. Mean time taken to stand was 194.1 + 247.1 seconds.

Conclusions: PD patients with early morning akinesia have abnormal movements when getting out of bed. As this is likely to be the first morning activity, difficulty in its execution can exacerbate other morning symptoms, for example, orthostatic hypotension, falls, and freezing of gait. Further studies are needed to clarify this problem and identify appropriate rehabilitation strategies for PD patients and caregivers.
