Comparative gait analysis of patients with vascular parkinsonism and idiopathic Parkinson’s disease

Objective: In this study we intended to characterize and compare the gait parameters of vascular parkinsonism (VaP) and Idiopathic Parkinson’s disease (IPD) patients using validated electronic walkway(GAITRite).

Background: VaP, is an entity characterized by combination of characteristic gait disorder and sub-cortical ischemic changes on neuro-imaging. VaP differs from IPD in clinical presentation, neuro-imaging, course of disease and response to levodopa. In some of the patients it becomes difficult to differentiate between IPD and VaP because of overlapping symptoms.

Methods: In this analytical cross sectional study, the gait parameters of patients with VaP (n= 21), those with IPD (n= 21) and healthy subjects (HS, n= 17) were estimated and compared using GAITRite®. Patients in all three groups (IPD, VaP and HS) were age and gender-matched.

Results: Gait-velocity was significantly slower among VaP patients as compared to IPD patients (VaP 45.53 Vs IPD 70.16 m/sec, p 0.001). The step count (VaP 31.67 Vs IPD 81.14, p 0.001), cadence (VaP 90.19 Vs IPD 105.21, p 0.006) and step length (right and left) were also less among VaP patients. VaP patients walk with a wider base as revealed by higher ‘heel to heel base support’ compared to IPD patients (left, VaP 14.40 Vs IPD 10.09, p 0.001). The fall predictors-double support time and stride length were higher in VaP patients. The toe in/out and cycle time differential were affected more among VaP group compared to IPD.

Conclusions: This study provides objective gait characteristics of VaP patients. The gait of VaP patients was slower and wider based as compared to IPD patients. The foot angle (the angle between the line of progression and the midline of the footprint) was also higher among VaP patients. The gait in VaP patients was more unstable and they had higher risk of fall as the fall predictors were higher among them than IPD. The findings of this study can be valuable in clinics. Slower velocity of gait, wider stance base and greater toe-out angle would favor VaP over IPD.
